The card features a grotesque kid character named Patient Zero, a humanoid figure made entirely of giant, cartoonishly-rendered viruses, bacteria, and wriggling tapeworms. The character has a basic kid shape, but every part is comprised of different colorful germs with angry little faces. The character is drawn with exaggerated, cartoonish features, with cilia for hair and flagella for fingers. The "clothing" is merely a hospital gown pattern drawn on the largest central virus. The background is a gradient of petri-dish beige to feverish red, with used tissues and fallen hypodermic needles scattered around. The card uses bold, graphic lines and garish, microscopic-view colors typical of Garbage Pail Kids. The "GARBAGE PAIL KIDS" logo is at the top, with "PATIENT ZERO" in a fuzzy, mold-textured banner at the bottom.
